Pa. girl doing well after 2nd lung transplant

PHILADELPHIA (AP) ? A 10-year-old Pennsylvania girl who underwent a double-lung transplant amid a national debate over the organ allocation process received a second set of lungs after the first failed, and has now taken some breaths on her own, the girl's parents said Friday.

The first set of lungs failed within hours of the June 12 transplant at Children's Hospital of Philadelphia, and Sarah Murnaghan was placed on machines, according to her mother. She was placed back on the lung transplant list the night after her surgery and received a second set of lungs on June 15.

"We were told ... that she was going to die," Janet Murnaghan said at a news conference Friday afternoon as she explained why Sarah's second transplant was not publicly disclosed. "We weren't prepared to live out her dying in public."

The suburban Philadelphia girl initially received lungs from an adult donor after her parents sued over national rules that place children behind adolescents and adults on the priority list for adult lungs ? even if the children are sicker and are capable of accepting adult organs.

The Murnaghans and the family of 11-year-old Javier Acosta of New York City challenged the policy making children under 12 wait for pediatric lungs to become available or be offered adult lungs only after adolescents and adults on the waiting list had been considered. Both children have end-stage cystic fibrosis.

A federal judge ruled they should be eligible for adult lungs after U.S. Health and Human Services Secretary Kathleen Sebelius declined to intervene.

Janet Murnaghan said Sarah's condition began to "spiral out of control" shortly after the first surgery. A second set of lungs was found and transplanted even though the donor once had pneumonia, making the surgery extra risky. The second set was also from an adult donor.

Sarah's mother said the second transplant was a success and the girl was able to take a few breaths on her own.

The failure of the first transplant is not uncommon. A 2005 University of Pennsylvania study found nearly 12 percent of lung transplants experienced what's called primary graft failure, where the organ almost immediately begins to fail.

But the timing ? she received a second set of lungs just three days after her first ? was narrow.

Of 5,081 lung transplants performed between 2010 and 2012, there were only seven retransplants within a week of the initial operation, according to the United Network for Organ Sharing, the private nonprofit group contracted by the government to manage the transplant list.

"I think this very clearly illustrates that the decision needs to be scientific and medical, rather than judicial," said Lawrence O. Gostin, a health law professor at Georgetown University who questioned the legal basis of the rulings. "UNOS was under pressure from the publicity surrounding this case and the court's decision. It is highly unusual to get two transplants within days."

According to UNOS, a graft failure does not automatically propel someone to the top of the waiting list of potential recipients, who have been assigned scores based on need. But Sarah's new score made her eligible for the second set of lungs.

Earlier this month, the Organ Procurement and Transplantation Network, which is overseen by UNOS, resisted making rule changes for children under 12 seeking lung transplants, but created a special appeal and review system to hear such cases. The special review option will expire on July 1, 2014, unless the full board of directors votes to keep it in place.

Of the 1,663 people currently seeking a lung transplant in the U.S., 12 are between the ages of 6 and 10.

Sarah was placed back on a ventilator due to partial paralysis of her diaphragm, a complication of surgery that is not allowing her lungs to expand her mother said.

Sarah is slated for surgery on Monday in an effort to repair her diaphragm.

"There's still a lot in front of us," Murnaghan said, but then added: "Sarah's a fighter. She's always been a fighter."

Scientists view 'protein origami' to help understand, prevent certain diseases

June 28, 2013 ? Scientists using sophisticated imaging techniques have observed a molecular protein folding process that may help medical researchers understand and treat diseases such as Alzheimer's, Lou Gehrig's and cancer.

The study, reported this month in the journal Cell, verifies a process that scientists knew existed but with a mechanism they had never been able to observe, according to Dr. Hays Rye, Texas A&M AgriLife Research biochemist.

"This is a step in the direction of understanding how to modulate systems to prevent diseases like Alzheimer's. We needed to understand the cell's folding machines and how they interact with each other in a complicated network," said Rye, who also is associate professor of biochemistry and biophysics at Texas A&M.

Rye explained that individual amino acids get linked together like beads on a string as a protein is made in the cell.

"But that linear sequence of amino acids is not functional," he explained. "It's like an origami structure that has to fold up into a three-dimensional shape to do what it has to do."

Rye said researchers have been trying to understand this process for more than 50 years, but in a living cell the process is complicated by the presence of many proteins in a concentrated environment.

"The constraints on getting that protein to fold up into a good 'origami' structure are a lot more demanding," he said. "So, there are special protein machines, known as molecular chaperones, in the cell that help proteins fold."

But how the molecular chaperones help protein fold when it isn't folding well by itself has been the nagging question for researchers.

"Molecular chaperones are like little machines, because they have levers and gears and power sources. They go through turning over cycles and just sort of buzz along inside a cell, driving a protein folding reaction every few seconds," Rye said.

The many chemical reactions that are essential to life rely on the exact three-dimensional shape of folded proteins, he said. In the cell, enzymes, for example, are specialized proteins that help speed biological processes along by binding molecules and bringing them together in just the right way.

"They are bound together like a three-dimensional jigsaw puzzle," Rye explained. "And the proteins -- those little beads on the string that are designed to fold up like origami -- are folded to position all these beads in three-dimensional space to perfectly wrap around those molecules and do those chemical reactions.

"If that doesn't happen -- if the protein doesn't get folded up right -- the chemical reaction can't be done. And if it's essential, the cell dies because it can't convert food into power needed to build the other structures in the cell that are needed. Chemical reactions are the structural underpinning of how cells are put together, and all of that depends on the proteins being folded in the right way."

When a protein doesn't fold or folds incorrectly it turns into an "aggregate," which Rye described as "white goo that looks kind of like a mayonnaise, like crud in the test tube.

"You're dead; the cell dies," he said.

Over the past 20 years, he said, researchers have linked that aggregation process "pretty convincingly" to the development of diseases -- Alzheimer's disease, Lou Gehrig's disease, Huntington's disease, to name a few. There's evidence that diabetes and cancer also are linked to protein folding disorders.

"One of the main roles for the molecular chaperones is preventing those protein misfolding events that lead to aggregation and not letting a cell get poisoned by badly folded or aggregated proteins," he said.

Rye's team focused on a key molecular chaperone -- the HSP60.

"They're called HSP for 'heat shock protein' because when the cell is stressed with heat, the proteins get unstable and start to fall apart and unfold," Rye said. "The cell is built to respond by making more of the chaperones to try and fix the problem.

"This particular chaperone takes unfolded protein and goes through a chemical reaction to bind the unfolded protein and literally puts it inside a little 'box,'" Rye said.

He added that the mystery had long been how the folding worked because, while researchers could see evidence of that happening, no one had ever seen precisely how it happened.

Rye and the team zeroed in on a chemically modified mutant that in other experiments had seemed to stall at an important step in the process that the "machine" goes through to start the folding action. This clued the researchers that this stalling might make it easier to watch.

They then used cryo-electron microscopy to capture hundreds of thousands of images of the process at very high resolutions which allowed them to reconstruct from two-dimensional flat images a three-dimensional model. A highly sophisticated computer algorithm aligns the images and classifies them in subcategories.

"If you have enough of them you can actually reconstruct and view a structure as a three-dimensional model," Rye said.

What the team saw was this: The HSP60 chaperone is designed to recognize proteins that are not folded from the ones that are. It binds them and then has a separate co-chaperone that puts a "lid" on top of the box to keep the folding intermediate in the box. They could see the box move, and parts of the molecule moved to peel the chaperone box away from the bound protein -- or "gift" in the box. But the bound protein was kept inside the package where it could then initiate a folding reaction. They saw tiny tentacles, "like a little octopus in the bottom of the box rising up and grabbing hold of the substrate protein and helping hold it inside the cavity."

"The first thing we saw was a large amount of an unfolded protein inside of this cavity," he said. "Even though we knew from lots and lots of other studies that it had to go in there, nobody had ever seen it like this before. We can also see the non-native protein interacting with parts of the box that no one had ever seen before. It was exciting to see all of this for the first time. I think we got a glimpse of a protein in the process of folding, which we actually can compare to other structures."

"By understanding the mechanism of these machines, the hope is that one of the things we can learn to do is turn them up or turn them off when we need to, like for a patient who has one of the protein folding diseases," he said.

Rye collaborated on the research with Dong-Hua Chen and Wah Chiu at the Baylor College of Medicine in Houston, Damian Madan and Zohn Lin at Princeton University, Jeremy Weaver at Texas A&M and Gunnar Schr?der at the Institute of Complex Systems in Germany.

Oily fish helps fight breast cancer | The Raw Story

Eating a portion of tuna, salmon, sardines or other oily fish once or twice a week reduces the risk of breast cancer, according to a review published by the British Medical Journal (BMJ).

Researchers based in China looked at 26 previously published studies covering more than 800,000 volunteers in the United States, Europe and Asia whose health was monitored and who gave details about their eating habits.

Oily fish is rich in so-called n-3 polyunsaturated fatty acids, or n-3 PUFAs, which are involved in the immune system, blood-vessel activity and chemical messaging in the brain.

The group of n-3 PUFAs has four members, known by their initials of EPA, DPA and DHA, which are mainly found in oily fish, and ALA, which is chiefly found in nuts, seeds and leafy vegetables.

The analysis showed that women with a high intake of n-3 PUFAs had a 14-percent reduction in risk of breast cancer compared with those who had a low intake.

But the protective effect came only for fatty acids that come chiefly from fish, and not for ALA acids.

Those who showed the most benefit were women in Asia, whose diet was richer in fish than in Europe and America.

In statistical terms, every 0.1-gramme increase in fish fatty acids per day was linked to a five-percent reduction in breast cancer risk.

As a guide for daily living, this means one or two portions of oily fish per person per week.

Breast cancer has been dubbed a ?silent killer? of women because it is often diagnosed too late.

The disease accounted for 23 percent of total cancer cases among women and 14 percent of cancer deaths in 2008, according to figures in the study.

Genetic heritage also plays a role in breast cancer, most notoriously in variants in the BRCA1 and BRCA2 genes, although how this interacts or not with food, lifestyle and environmental factors is unclear.

The first floor of the house qualified for home office deductions. It was physically separated from the rest of the house and had a separate entrance and awning.

But the attorney didn?t meet his ?burden of proof? for the apartment deductions. He did not offer evidence how the computer room was used nor did he substantiate his claim that the sun room was used to store documents and supplies. Oil price stays above $97 on improving US data

BANGKOK (AP) ? The price of oil rose Friday as the U.S. economic outlook brightened and concerns eased about a credit crunch in China.

Benchmark oil for August delivery was up 20 cents to $97.23 per barrel at midday Bangkok time in electronic trading on the New York Mercantile Exchange. The contract rose $1.55 to finish at $97.05 on Thursday after the U.S. government released data showing an increase in in consumer spending and home sales while jobless claims fell.

"The truth to these markets are that they are lacking any good reason to go down," said Carl Larry of Oil Outlooks and Opinions. "We're starting to settle into a fair value and it's hard to argue lower."

The number of Americans seeking unemployment benefits fell by 9,000 to a seasonally adjusted 346,000 last week, evidence that the job market is still improving modestly. Steady job gains could help the economy expand later this year and thus increase energy consumption.

The U.S. Commerce Department said consumers spent more in May as their income rose, although spending was weaker in April, February and January than previously estimated. The number of pending home sales jumped in May to the highest level in more than six years, the National Association of Realtors said.

Fears of a cash crunch in China, which could cripple small- and medium-sized businesses, were eased after the country's central bank indicated it would not allow an all-out crisis to unfold.

Analysts at Bank of America Merrill Lynch said in a commentary that "it is time for markets to calm down now and that the worst is probably behind us" and that a hard economic landing would be avoided, since no Chinese policymaker would want to shoulder the blame for that.

Brent crude, which is used to set prices for oil used by many U.S. refineries to make gasoline, rose 18 cents to $103 a barrel.

In other energy futures trading on the Nymex:

? Heating oil rose 0.7 cent to $2.895 a gallon.

? Natural gas rose 0.4 cent to $3.586 per 1,000 cubic feet.

? Wholesale gasoline rose 0.1 cent to $2.729 a gallon.

2nd man held in slaying probe as Hernandez challenges bail ruling

Former New England Patriots tight end Aaron Hernandez will remain in jail after Superior Court Judge Renee Dupuis denied his bail appeal.

Authorities are investigating whether Aaron Hernandez, the NFL star accused of murder in the recent shooting death of a friend, was involved in the drive-by killings of two men last year, sources told NBC News on Thursday.

The men, Daniel Abreu and Safiro Furtado, were shot to death from an SUV on July 16, 2012, after leaving a Boston nightclub.

Police put out a description of the SUV but never made an arrest. Abreu was driving in a BMW, and Furtado was in the passenger seat. One of three people in the backseat was also shot and survived.

Hernandez was charged Wednesday with first-degree murder in the execution-style killing earlier this month of Odin Lloyd, whose body was discovered in an industrial park not far from Hernandez?s home. Hernandez, an All-Pro tight end, was released by the New England Patriots after his arrest.

Prosecutors said he had summoned two friends from out of state before driving to pick up Lloyd and carry out the killing.

On Thursday, Hernandez was denied bail for a second time. Superior Court Judge Renee Dupuis said that the state?s case appeared ?circumstantial but very, very strong.? Hernandez was appealing a denial of bail by another judge the day before.

Prosecutors said they had uncovered four new pieces of evidence in less than 24 hours after searching a condo leased by Hernandez. They said they had found ammunition, a clip and a picture of Hernandez with a Glock handgun.

William McCauley, an assistant district attorney, also said that Hernandez had interfered with the investigation by home surveillance-camera video and instructing his girlfriend not to talk to investigators.

?The evidence of his guilt is overwhelming,? prosecutor William McCauley said.

Hernandez?s lawyers argued that he deserved bail because of his upstanding character and clean record, and because he was not a risk to flee. They noted that he stayed put last week, when rumors circulated in the media that Hernandez was about to be arrested. The judge was unmoved.

Considering the details of the case, ?The idea that I could release him on a bracelet and he would comply with court rules is not something that I am willing to accept,? Dupuis said. ?A bracelet just wouldn?t keep him here. Nor would $250,000.?

Hernandez watched from along one wall of the courtroom, standing behind a partition and shackled at the wrists. He has pleaded not guilty to the first-degree murder charge and five gun charges.

Meanwhile, authorities in Connecticut announced they had charged a second man in connection with Lloyd?s killing ? Carlos Ortiz of Bristol, the city where Hernandez grew up. Ortiz was charged as a fugitive from justice, appeared in court and agreed to return to Massachusetts, said Brian Preleski, the state?s attorney for New Britain.

Ortiz?s connection to the investigation was unclear, and the prosecutor gave no other details.

Prosecutors in Massachusetts laid out a detailed account of Lloyd?s killing on Wednesday but did not say who fired the fatal shot. They said that Hernandez was apparently upset that Lloyd, three nights earlier at a nightclub, was talking to people Hernandez had problems with.

Hernandez, 23, was being held at Bristol County Jail in Dartmouth, Mass., where the sheriff said he would be treated like any other inmate ? no workout equipment, no TV, no Internet access and, on the first night, spaghetti for dinner.

Prosecutors say Hernandez was still fuming from the nightclub dispute three days earlier when he orchestrated the killing June 17.

They said Hernandez and the two friends picked Lloyd up at his house at 2:30 a.m. Surveillance footage from Hernandez?s house shows him leaving earlier in the night with a weapon, they said.

They said Lloyd got into the car and texted his sister, ?Did you see who I am with,? and later added, ?NFL? and ?Just so you know.? Prosecutors said Thursday that they believe Lloyd sent the texts because he was concerned for his safety.

Later that morning, between 3:23 a.m. and 3:27 a.m., workers on the overnight shift at the industrial park reported hearing gunshots, authorities said. It was not clear who investigators believe fired the shots.

Prosecutors say security videos from Hernandez?s house show him with firearms after Lloyd was murdered and show a Nissan Altima ? the same type of car Hernandez had rented ? coming and going at the industrial park.

Surveillance footage captured Hernandez getting out of the car at his house at 3:29 a.m. with a gun, prosecutors say.

The Patriots cut him within hours after he was led from his home in handcuffs. The team had given Hernandez a five-year, $40 million contract last summer, including a $12.5 million signing bonus. The investigation also cost Hernandez his endorsement deal with CytoSport, the maker of the Muscle Milk supplement drink.

Though the Elise, Exige, and Evora earned respect from legions of hardcore driving enthusiasts, Lotus hasn't exactly endeared itself as an automotive brand of late. Following the fizzled promise of a ridiculously ambitious lineup?and the ousting of CEO Dany Bahar, the brand has found some solace in racing-- though some have accused even those efforts of being driven more by marketing than outright gearhead passion.?

Enter Lotus's latest chapter, which sees the fabled brand announcing their imminent entry into the motorized two-wheeled realm. By using the resources of German racing firms Kodewa and the Holzer Group, Lotus says their new C-01 motorcycle "... will reflect a combination of lifestyle, design and high end technology" utilizing materials like "... carbon, titanium and aerospace quality steel, which are also used in Formula 1."

The future-friendly "hyper bike" will incorporate the curious juxtaposition of body shapes inspired by the legendary Lotus 49 race car and an engine producing somewhere in the neighborhood of 200 horsepower. Design is said to come from futurist Daniel Simon, who was responsible for the light cycles in Tron: Legacy, and Lotus suggets they'll be teasing images of the impending bike sooner rather than later.

While the idea of a lightweight, ultra high horsepower motorcycle with retro styling certainly sounds tasty, we also hope these bikes are more than mere?pie-in-the-sky publicity stunts. After all, Lotus founder Colin Chapman has already performed enough revolutions in his grave.

Williams apologizes again after rape case comments

(AP) ? Serena Williams has issued a new apology for what she called "insensitive and misinformed comments" she made about the victim in the Steubenville, Ohio, rape case.

In a Rolling Stone story posted online last week, Williams said the 16-year-old girl "shouldn't have put herself in that position."

The next day, Williams apologized in a statement. She also apologized at a news conference Sunday.

On Monday night, a day before Williams plays her first-round match at Wimbledon, she released another statement on her website, saying she also wanted to "extend an apology to the millions of sexual assault victims in America whose pain could have been compounded by my inappropriate remarks." Sexual assault, Williams wrote, "is never acceptable and never the fault of the victim."
